UPDATE 6/23/2020 – 8:55 AM – The June 29th event has been canceled by the service providers. Athletes will still need to have a current physical on file with us by July 15th. Please contact your primary care physician to schedule an appointment. Also, we will still need the Rank One forms filled out.

Cleveland Athletics in cooperation with Cleveland Area Hospital and Lake Area Medical Associates have arranged a night for athletes to get their physicals, as well as fill out the necessary medical release forms for the 2020- 2021 school year. 

Rank One is our student information and medical release forms portal for athletics. Parent instructions for Rank One are listed below for those who have internet access. For anyone who does not have internet access hard copies will be available.  For the safety of our staff, athletes and medical providers we will have social distancing protocols in place.

Physical $15.00, Rank One Fee $10.00
