Bellow are the forms we will need to help you get your student enrolled. They are being offered here if you would like to fill the forms out prior to coming to the districts’ main office. Currently, you will need to download, print and return the packet to the district offices in person or by mail it along with the other documents requested to complete the process.
In no way is this meant to be the only way to enroll a new student. You may also come by the administration office and enroll in person during normal business hours and we will be glad to assist you.
Also, If you would like to request the packet be mailed to you ahead of time, please call (918) 358-2210 and select option 1 or send a request by email and we will get it to you as soon as we can.
Addendum: For school enrollment, a parent or guardian shall provide one of the following:
- 1. Current, up-to-date immunization records; or
- 2. A completed and signed exemption form.
